2008-2009 Hockey Initiation REGISTRATION FORM
LEARN TO SKATE
Program teaches recreational and/or hockey skating to ages 4 to adult and is a great way to
improve your skating skills. It can ready you to play hockey, figure skate or speed skate.
$68 each 8 week session, sessions run all year round. Sessions begin September 16th 6pm or 6:40pm classes.
Learn to Play Hockey
Also known as the Hockey Initiation Program (HIP) is for boys and girls of all ages and is designed for
beginning hockey players who want to learn the basics of the game. The players should
know skating basics. We will continue to focus on improving skating skills each session,
teach stick-handling, positioning, and have cross-ice scrimmage games as the season
progresses, depending on skill improvement. Our goal at the HIP level is to have taught
hockey skills and improve skating so that in the player’s second year of skating they
may progress to the De Pere or Green Bay Mite Hockey programs. This is great place to get started.
If you have a older child wishing to start hockey we will place them here and move them into the age and skill appropriate recreation level at the appropriate time .

Equipment Rental
Green Bay District Skaters The rink has 30 sets of equipment on a first come first served basis. Call Don Chilson at 403-2000.
Equipment rental includes helmet, shin pads, elbow pads. Additional equipment needed includes skates, gloves and breezers (pants).
DePere District Skaters The DePere Youth Hockey Association has equipment (excluding skates) for you.

Instructions for On-line Registration with USA Hockey
It is mandatory that all players and coaches register on-line with USA Hockey. Players should attache their printed registration form to thier registration form and mail it to 1640 Fernando Dr. Depere, WI 54115. See below for instructions on registering with USA Hockey When Registering, 6 and Under are free, 7 and up are $40, This includes the insurance program and USA Hockey Magazine for your player.
The on-line registration process is simple and should take you no longer than 5 minutes to complete.
1. Go to www.USAhockey.com
2. Click on the quick link in the red box that says “Become a USA Hockey member”
3. Simply follow all of the prompts to register. Make sure to use the full legal name (no nicknames or shortened version of the name)
4. You will need a credit card to make payment of $40 ( $30 USA Hockey $10 WAHA fees)
5. At the end of the on-line registration process it will ask you to open confirmation and print. This is the page you will need to send in with your registration!!
If you are unable to attend one of the open registration nights, it is your responsibility to register on line and make sure your form is received with registration.
